Analysis and practice development

The team have many years experience in working with general practices to improve systems that will lead to safer and more effective prescribing.  These include for example:

• Repeat prescribing system development
• Systems for safer prescribing e.g. methotrexate and warfarin.
• Clinical medication review
• Formulary development
• Prescribing data analysis
• Commissioning of medicines management services

Audit – risk reduction, financial savings and reducing health inequalities

We have an extensive set of audit protocols that have been tested and modified over the years. These cover the following areas:

• Reducing prescribing risk e.g. NSAIDs in older people
• Improving the cost effectiveness of prescribing e.g. medicine switches
• Implementation of NICE guidelines and TAGs
• Addressing health inequalities - identified under prescribing in areas such as cardiovascular disease, diabetes and osteoporosis

Patient level services

We have pharmacists with a specialist interest (PWSI) who are accredited to run anticoagulant services.  We also have a specialist interest in prescribing for older people and provide services to care home residents and older people in their own homes.
All pharmacist members of the team are qualified Independent Prescribers.
